2026-27 Enrollment FormWe are now enrolling Pre-K and kinder students for fall of 2026. Pre-K students must be 4 years old on or before September 1st to enroll. In addition to a completed enrollment form, we will need a copy of their birth certificate, immunization record and proof of Middleberg district residence. Our PreK enrollment is on a first come, firs serve basis. If you have any questions, please call the school office.
